In an effort to protect our patients and team during the global outbreak of Coronavirus, we have made the decision to close our clinic as of March 19th.
We will continue to follow the recommendations of the Canadian and Ontario governments in the coming days and keep you informed of any changes. It is our duty to actively participate in slowing the spread of the COVID-19 virus.
If you have an upcoming appointment, we will be in touch shortly with rescheduling options. During this time, Dr. O’Neill may offer a virtual consultation for our patients for follow up, as needed, in the safest manner possible.
Virtual Care Privacy Statement: Just like online shopping or email, Virtual Care has some inherent privacy and security risks that your health information may be intercepted or unintentionally disclosed. We want to make sure you understand this before we proceed. In order to improve privacy and confidentiality, you should also take steps to participate in this virtual care encounter in a private setting and should not use an employer’s or someone else’s computer/device as they may be able to access your information.
If it is determined you require a physical exam you may still need to be assessed in person. You should also understand that virtual care is not a substitute for attending the Emergency Department if urgent care is needed.

- A photo of your OHIP (health card)
- 3 pictures of the area of concern
- Best contact phone number to reach you
- The FAX Number of your pharmacy
- Please write: I have read the privacy statement above, and hereby give consent to Dr. O’Neill for virtual visit and electronic communication
For prescription refills please contact your pharmacy and they will fax the request to us, and we will respond.
